Since 1990, the Health Services Center has served hundreds of people living with HIV and AIDS, as well as their families. Our main office is located in Hobson City, near Anniston.

CLINIC LPN 

  • LPN Degree with experience in HIV Medical Care preferred. 
  • Conduct in house labs
  • administer vaccinations and injections
  • Perform exams and patient assessments
  • Assist and back up phlebotomy
  • Assist providers and nurses with dat-to-day clinic flow and patient visits 
  • Document patient encounters and medical visits in EMR system
  • Vital signs and triage back up 
  • Satellite clinic travel 
  • Telehealth visits with patients 
  • Schedule and follow up with referrals

CASE MANAGER

  • Provide daily task as necessary for patient case management 
  • Follow-up with patient appointment to ensure adherence 
  • Outreach to patients who have fallen out of care to ensure retention and adherence
  • Outreach to newly diagnosed patients to ensure retention and adherence 
  • Chart appointments, follow-ups and other contact in a timely and appropriate manner in the EMR system 
  • Travel to satellite clinics within the 14 county coverage area as needed and required by providers 
  • Link HSC patients with other resource in the community to meet identified unmet needs
  • Collaborate with other HSC professionals to ensure seamless care delivery

ADAP/AIAP Case Manager

  • Provide daily tasks as necessary for patient ADAP/AIAP eligibility and enrollment
  • Follow-up with patient appointments to ensure adherence and retention 
  • Outreach to patients who have fallen out of care to ensure retention and adherence 
  • Outreach to newly diagnosed patients to ensure retention and adherence
  • Chart appointments , follow-ups and other contact in a timely and appropriate manner in the EMR system 
  • Link HSC patients with other resources in the community to meet identified unmet needs 
  • Collaborate with other HSC professionals to ensure seamless care delivery

Clinic Social Worker

  • Degree in Social Work required. Licensure in Social Work required. 
  • Provide daily task as necessary for patient case management 
  • Conduct patient intake and screening to determine appropriate placements 
  • Development of patient case plans
  • Update and reevaluation of patient case plans every six months
  • Travel to satellite clinic s within the 14 county coverage area as needed and required by providers
  • Chart interviews, assessments and other contact in a timely  and appropriate manner in the EMR system 
  • Patient referrals as ordered by providers
  • Link HSC patients with other resources in the community to meet identified unmet needs
  • Collaborate with other HSC professionals to ensure seamless care delivery
