We are a 501 c3 registered non-profit, Community Based Organization (CBO) and medical clinic providing HIV/AIDS medical care, education, and support to a fourteen county area of East Alabama. Since 1990, the Health Services Center has served hundreds of people living with HIV and AIDS, as well as their families. Our main office is located in Hobson City, near Anniston.

SA Counselor 

  • Conduct substance abuse assessments (see procedural manual) to determine appropriate placements
  • Facilitate psychotherapeutic/educational groups for substance abuse treatment program
  • Assist with required grant initiatives and presentations related to the HSC substance abuse treatment program
  • Complete and maintain accurate records or reports regarding the patients’ histories and progress, services provided, or other required information.
  • Interview clients, review records, and confer with other professionals to evaluate individuals’ mental and physical condition and to determine their suitability for participation in a specific program and complete them in a timely and appropriate manner.
  • Develop client treatment plans based on research, clinical experience, and client histories.
  • Review and evaluate clients’ progress in relation to measurable goals described in treatment and care plans.
  • Write reports and evaluations
  • Link HSC clients with other resources in the community to meet identified unmet needs
  • Collaborate with other HSC professionals to ensure seamless care delivery
  • Store charts and other client information in locked file cabinets and/or locked cases only accessible to the Substance Abuse Counselor and NO ONE ELSE.
  • Ability to learn and work on an EMR (Electronic Medical Record) system
  • Must be willing to obtain at own cost your ALC, LPC, or Alabama Alcohol and Drug Abuse Association certification prior to your second year of employment with the agency.

CASE MANAGER

  • Provide daily task as necessary for patient case management 
  • Follow-up with patient appointment to ensure adherence 
  • Outreach to patients who have fallen out of care to ensure retention and adherence
  • Outreach to newly diagnosed patients to ensure retention and adherence 
  • Chart appointments, follow-ups and other contact in a timely and appropriate manner in the EMR system 
  • Travel to satellite clinics within the 14 county coverage area as needed and required by providers 
  • Link HSC patients with other resource in the community to meet identified unmet needs
  • Collaborate with other HSC professionals to ensure seamless care delivery

ADAP/AIAP Case Manager

  • Provide daily tasks as necessary for patient ADAP/AIAP eligibility and enrollment
  • Follow-up with patient appointments to ensure adherence and retention 
  • Outreach to patients who have fallen out of care to ensure retention and adherence 
  • Outreach to newly diagnosed patients to ensure retention and adherence
  • Chart appointments , follow-ups and other contact in a timely and appropriate manner in the EMR system 
  • Link HSC patients with other resources in the community to meet identified unmet needs 
  • Collaborate with other HSC professionals to ensure seamless care delivery

Clinic Social Worker

  • Degree in Social Work required. Licensure in Social Work required. 
  • Provide daily task as necessary for patient case management 
  • Conduct patient intake and screening to determine appropriate placements 
  • Development of patient case plans
  • Update and reevaluation of patient case plans every six months
  • Travel to satellite clinic s within the 14 county coverage area as needed and required by providers
  • Chart interviews, assessments and other contact in a timely  and appropriate manner in the EMR system 
  • Patient referrals as ordered by providers
  • Link HSC patients with other resources in the community to meet identified unmet needs
  • Collaborate with other HSC professionals to ensure seamless care delivery
